Essentia Health - Graceville Clinic is located in the South Wing of Holy Trinity Hospital and offers services six days a week. GHC Clinic is staffed Monday through Saturday and a clinic provider offers on-call care on Sundays and during holidays. Grace Home is a 45-bed long term care facility built on strong Christian foundations. These foundations provide the strength and vision to meet the challenges of the future through faith and belief in the dignity and uniqueness of each individual. Grace Home was founded to provide physical, spiritual and social care for individuals requiring supervised daily living as well as varying degrees of nursing services in a home-like setting.

Grace Village is a 16 apartment Assisted Living Facility. It allows senior citizens who are unable to stay at home but do not need skilled nursing care, to move into a residential setting with services designed to meet their needs. In addition to being less expensive than skilled nursing care, Grace Village provides tenants with private apartments rather than an institutional setting.
